Supporting your independence
How we can help
We work with your goals not a generic programme. Wherever a neurological condition is affecting your life at home, that’s where we focus.
Staying mobile and strong
Maintaining the movement, balance, and physical strength that keeps you safe, confident, and active at home and reduce your risk of falls.
Managing everyday activities
Supporting your ability to dress, prepare meals, get out and about, and keep doing the everyday things that matter to you.
Communication and thinking
If a stroke or neurological condition has affected your speech, memory, or thinking; we help you find ways to stay connected and express yourself.
Emotional wellbeing
Adjusting to life with a neurological condition is hard for you and for your family. Our team supports you through the emotional side of that journey too.
Our clinicians work alongside you to help you:
- Stay safely in your own home
- Improve strength and balance
- Recover after illness, injury or a hospital stay
- Manage pain, fatigue and chronic symptoms
- Increase confidence with everyday activities
- Support thinking, memory and communication
- Remain connected with your community
- Maintain your independence for as long as possible
Our interdisciplinary team
Unlike clinics where each discipline works separately, our team communicates and collaborates around your goals as one unit. One referral covers everything, you don’t need to manage five different providers.
- Physiotherapy
- Occupational Therapy
- Speech Pathology
- Exercise Physiology
- Psychology
- Social Work

Frequently Asked Questions
Do I need to change my package provider to work with you?
No. You keep your existing package provider, the organisation that manages your Support at Home funding. You simply nominate ONE Rehab as your preferred therapy provider, and we set up the service arrangement directly with them. Nothing else changes.
What if I'm not sure whether therapy is covered under my funding?
Allied health therapy including physiotherapy, occupational therapy, speech pathology, exercise physiology, and psychology; sits within the Clinical Care category under Support at Home, which is fully funded by the Australian Government. If you’re unsure about your specific situation, we’re happy to help you work through that with your package provider.
Do I need a GP referral to get started?
No GP referral is needed to book a free introductory session. If your funding requires any additional documentation, we’ll let you know after your first meeting and help you work through it.
I'm not yet on Support at Home, can I still contact you?
Yes. If you think you or a loved one may be eligible for Support at Home funding, the first step is to contact My Aged Care on 1800 200 422 or visit myagedcare.gov.au to request an assessment. In the meantime, you’re welcome to reach out to us, we’re happy to talk through your situation and help you understand whether we could be the right fit.
For package providers and care coordinators
Working with us as an associated provider
We are an exclusively neurological allied health clinic. Every clinician, every programme and every piece of equipment in our service is dedicated to supporting people living with stroke, brain injury, Parkinson’s disease, multiple sclerosis and related conditions. If your client has a neurological condition and Support at Home funding, we offer one of Adelaide’s most focused and experienced neurological allied health services.
- Exclusively neurological. Not a generalist service adapted for aged care, a tailored clinic built from the ground up for this population.
- Interdisciplinary, not multidisciplinary. Our team works as one unit around your client's goals. One referral, one team; not five separate providers for your coordinator to manage.
- We manage the associate provider arrangement. We're experienced with Support at Home service agreements and handle the paperwork directly with your team.
- Clear clinical reporting and communication. Timely progress reports, collaborative care planning, and responsive communication as standard; not as an afterthought.
- Simple, single-stream referral. One referral covers all disciplines. No separate referrals per clinician type.
